To get started fill out the Consent to Treat form and New Patient Intake forms below, in “How did you hear about us” put “Blue Jug Health Market Client”.
Once you submit your forms, call 409-210-3336 and tell them you’re a Blue Jug Health Market client who just submitted paperwork and you need to schedule a televisit for the Medically Managed Weight Loss Program. They should have you scheduled within 48 hours.
Once you are approved for this program you will begin your injections with Blue Jug Health Market.
